On Linux you create USB bootable image as simple as:

pv < memtest86-usb-v9.0-pro.img > /dev/sdX

- Run this as root, of course;
- X is your USB drive letter;
- If you do not use pv, use dd, or whatever;
- It may boot at this point, but I recommend
  running gdisk /dev/sdX and only w as for write;
  it fixes some backup header. Have fun!
